After the presentation, please sign the Google Form that lets us know you completed the video. (see below) Please fill out the entire form.
After signing the Google Form below, the office staff will get you added to our volunteer system and then you will be notified by email on how to proceed with the process. For those that will just be a visitor, you will be required to provide your Driver's License in the office to be scanned. For those that will be in the classroom, or chaperoning a day-time field trip or overnight trip, you will be required to create a user portal and pay for a background check with Raptor Technologies. You are responsible for the payment which is $15 and can be paid online thru your portal. This $15 payment is good for one school year and must be renewed every year after that. After you have signed up thru the Raptor website, ,please make sure to fill out this waiver and turn in to the office as well. PARENT WAIVER
Once these steps are completed you will be notified from Lebanon CUSD #9 that your background check is approved and that you can now start volunteering in our district.
