REGISTRATION HELD FOR LEBANON DIST. #9


Registration for the 2019-20 school year will be held on Thursday, June 20, 2019 from 12:00 p.m. to 7:00 p.m., and on Monday, June 24, 2019 from 12:00 p.m..to 7:00 p.m..

ALL grades will register at Lebanon Elementary School. If you have any questions, please feel free to call LGS at 537-4553 or JrH and LHS at 537-4423.

Items Required for Registration

Proof of Residency:

1. The first form of “Proof of Residency” should be a

- Most recent property tax bill and proof of payment (cancelled check or

Form 1098)

- Mortgage papers (homeowners)

- Signed and dated lease and/or proof of last month’s payment (cancelled check or receipt (renters))

- Letter of residency from landlord in lieu of lease

2. The second and third form may be one of the following:

- Driver’s license showing current address

- Vehicle registration showing current address

- Voter registration showing current address

- Most recent cable television and/or credit card bill with current address

- Current public aid card with current address

- Current homeowners/renters insurance policy and premium payment receipt

- Most recent gas, electric and/or water bill

- Current library card

- Receipt for moving van rental

- Mail received at new residence

Certified Birth Certificates:

Required for all students new to the district

Health Forms:

Illinois Health Physical Form

Students new to Illinois must have a physical completed on a State of Illinois Health Physical Form.

All Pre-K, Kindergarten, 6th, and 9th grade students must have a recent physical. (Sports physicals must be done yearly.)

All students must be up-to-date on immunizations (bring shot record)

Transfer Forms:

Parents of all new students will be required to complete a Release of Records Form required from the last school attended. If the student is coming from another Illinois school the previous school must provide a Student Transfer Form.

STUDENT PHYSICAL EXAMINATIONS

State Law in Illinois requires that all students entering Pre-Kindergarten, Kindergarten, sixth and ninth grades have a physical examination. Students in Kindergarten, second and sixth grades are required to have dental examinations. Students in Kindergarten and students first entering into an Illinois school beyond Kindergarten are required to have an eye exam by a licensed optometrist or medical doctor who performs complete eye examinations.

Ninth grade is now required to have a dental exam completed.

The new school year will begin for students in grades 1 through 12 on Wednesday, August 14, 2019. Students in Kindergarten will begin on Friday, August 16, 2019. All classes will be in session for the FULL day. Breakfast and lunch will be served beginning that date.

FREE AND REDUCED LUNCH RATES

Information about the Lebanon School District policy for free or reduced price meals for families unable to pay the full price for meals under the National School Lunch and Breakfast Program will NOT be available during school registration. Information will be sent home with students at the beginning of the school year. Students should be prepared to pay for their breakfast/lunch until the application is approved.

Questions may be directed to the appropriate school office, where applications may also be obtained.

If your student received free/reduced lunches at the end of the 2018-19 school year, they will have those benefits extended thru the first 30 days of the 2019-20 school year which would be thru September 24, 2019. After that date, your student will be dropped from free/reduced lunches unless a new application has been filled out and turned in or they meet the criteria of direct certification from the State of Illinois.

For school year 2019-20, lunch prices have been set at $2.20 for Elementary students and $2.40 for Jr. High and High School students.


LAW TIGHTENS ADMINISTRATION OF

DRUGS IN SCHOOL

Due to changes in law, Lebanon schools are no longer permitted to dispense ANY medication, whether by prescription or over-the-counter type, without the signature of both a licensed prescriber and a parent.

This means that students may no longer get any acetaminophen, ibuprofen, or cough drops with only parental permission. Now a “licensed prescriber’s” signature is also required.

PAY BUS IS AVAILABLE

The Lebanon School District plans to have a “pay” bus available to pick up students at designated pick-up points in Lebanon. This transportation is for students who live within one and a half miles of the school and do not qualify for free transportation.

The cost is $25.00 per month per child not to exceed $50 per family.

TITLE IX RULES

Equal educational and extracurricular opportunities shall be available for all students without regard to race, color, national origin, sex, religious beliefs, physical or mental handicap or disability, status as homeless, or actual or potential marital or parental status, including pregnancy.

No student shall be denied equal access to programs, activities, services, or benefits or be limited in the exercise of any right, privilege, advantage or be denied access to educational and extracurricular programs and activities on the basis of sex, race or religion.

It is illegal and against school board policy for any employee, students, or other person, male or female, to sexually harass any employee or student on school property or during school activities or as a result of the employment or educational relationship.

All district schools are accessible to individuals with disabilities. Anyone requiring special accommodations including, but not limited to parent conferences, school programs, or school board meetings, is invited to contact the district office.

TITLE IV FUNDS

The Lebanon School District receives funding under Title IV to support programs to promote safe, drug-free schools. Examples of such programs include D.A.R.E. and Red Ribbon Week.

TEXTBOOK RENTALS AND FEES

Textbook rental and fees need to be paid PRIOR to the first day of school. Fees for the 2019-20 school year will be:

Kindergarten $85.00

Elementary $85.00

Junior High $95.00

High School $95.00

Science Class Fee $30.00

Horticulture/Landscaping $30.00

Art/Painting Class Fee $30.00

Digital Photography $40.00

Band Fee $50.00

Drivers Ed Fee $150.00

District-Wide Technology Fee $50.00


Reduced Book Rental Rates will be for elementary (K-5) - $45.00; for Jr. High and High School (6-12) - $50.00. Reduced District-Wide Technology Fee - $25.00

Junior High $40.00 per sport

High School $60.00 per sport

Please reference your student’s school handbook when they get them during their first week of school. Policies and procedures regarding this matter will be addressed in the separate handbooks.



VOLUNTEER PROGRAM AT

LEBANON ELEMENTARY SCHOOL

Lebanon Elementary School initiated a parent and citizen volunteer program several years ago, which will continue during the upcoming school year because of the success it enjoyed previously.

The purpose of this program includes promoting a strong partnership between the home and school, better meeting the individual needs of students, providing teachers more instructional time and expanding the resources of the school through citizen contributions.

Volunteers assist in the school by tutoring and supervising students, performing clerical tasks, servicing as resource persons, assisting teachers in a variety of other ways, and coaching.

Anyone interested in volunteering or has questions may contact the elementary school principal.

FRESHMAN & NEW HIGH SCHOOL STUDENTS

ICE CREAM SOCIAL

There will be an Ice Cream Social and Tour of Lebanon High School on Tuesday,

August 6, 2019, in the LHS Commons, from 10:00 – 11:30 a.m. and is sponsored by the LHS National Honor Society. This will be a time to come and ask questions about your schedule, check out your lockers, go on a tour, and discover what clubs and organizations there are at Lebanon High School. This will give you an opportunity to make your first day great!

OPEN HOUSE

Open House for District 9 will take place on Monday, August 12, 2019 from 5:00 p.m. to 6:30 p.m. This will give your child an opportunity to get familiar with new classrooms and teachers. Students will be able to bring in their school supplies that evening to give to their teachers or to put in their lockers.

HS STUDENTS THAT DRIVE TO SCHOOL NEED TO REGISTER FOR PARKING SPACES

All HS students that drive to school MUST park in the student parking lot. In order to do so, you must register your spot in the HS Office by providing copies of your driver’s license and proof of insurance. Students can register for parking spots at registration or the first two days of school being is session.
