ADMISSIONS

 

The following requirements for enrollment in the Lebanon Schools, are more fully described in the district’s student residency policy No. 7:60 available at the District Office:

  1. The student must legally reside in Lebanon District #9 with a parent or legal guardian.
  2. Three items showing proof of residency as defined below.
  3. Forms from previous schools as listed below.
  4. When a child loses permanent housing and becomes a homeless person as defined by law, or when a homeless child changes his or her temporary living arrangements, the parent or guardian of the homeless child has the option of either:

-continuing the child’s education in the school of origin for as long as the child remains homeless or, if the child becomes permanently housed until the end of the academic year during which the housing is acquired; or

-enrolling the child in any school that non-homeless students who live in the attendance area in which the child or youth is actually living are eligible to attend. 

 

Items Required for Registration

 

Proof of Residency:

 

Three (3) forms of Proof of Residency are required for every NEW and RETURNING student.

 

1. The first form of "Proof of Residency" should be a

 

_ Most recent property tax bill and proof of payment (cancelled check or Form 1098)

_ Mortgage papers (homeowners)

_ Signed and dated lease and proof of last month’s payment (cancelled check or receipt (renters))

_ Letter from manager and proof of last month’s payment (cancelled check or receipt (trailer park

residents))

_ Letter of residence from landlord in lieu of lease

.

2. The second and third form may be one of the following:

 

_ Driver’s license showing current address

_ Vehicle registration showing current address

_ Voter registration showing current address

_ Most recent cable television and/or credit card bill with current address

_ Current public aid card with current address

_ Current homeowners/renters insurance policy and premium payment receipt

_ Most recent gas, electric and /or water bill

_ Current library card

_ Receipt for moving van rental

_ Mail received at new residence.

 

Certified Birth Certificate:

Required for all students new to the district

 

Health Forms:

Illinois Health Physical Form

Students new to Illinois must have a physical completed on a State of Illinois Health Physical Form.

All Pre-K, Kindergarten, 6th grade, and 9th grade students must have a recent Physical. (Sports physicals must be done yearly.)

Students should be up-to-date on immunizations (bring shot record)
